Under article 10 of the Constitution, all citizens shall have dignity and value, and all citizens shall possess the right to pursue happiness.The country shall hold the responsibility of confirming and ensuring these individual, inviolable basic human rights.

I hereby confirm that the above was written with the comprehension that I possess the right to refuse the collection of my personal information above and that, in the event that I do refuse, I will be unable to receive both consultation and smooth assistance for incident handling.​

· Notification shall be provided and agreement shall be obtained before using this information for any purpose outside of the goals of collection or before providing the information to a 3rd party.​

· Please be aware that the filer of any report that is categorized as a whistleblowing report or a report of corruption will be protected under the Whistleblower Protection Act.​
